Reporting surgery-related injuries

Surgeons and clinicians are encouraged to report any significant injuries that occur as a result of, or in association with, an operative procedure.

How to report

  1. Fill out the Reporting surgical morbidity form (below)
  2. Submit directly to the Victorian Surgical Consultative Council (VSCC) Chairperson.

All identifying information in these forms is treated as confidential. The Chairperson may contact you if additional information is required.
